Chronic Conditions Management

 

There is a rapid worldwide increase in the number of people whose lives are affected by a chronic condition. This expansion of chronic need will impact on health systems globally, and will be of such a magnitude that health services as they are currently organised and structured will not be able to respond appropriately.

 

The Minister of Health has identified as a priority, the need to control the burgeoning chronic disease burden. The burden of chronic diseases is a major risk for the viability and sustainability of the New Zealand health system as well as for NMDHB and this risk will increase as the population ages and as technology continues to advance.

There will be up to 50 different Community Health/Wellness Providers informing others about the work they do in the Nelson Tasman region.

 

More and more, the evidence shows that the best health outcomes are achieved when a holistic view of health is taken, biomedical as well as psychosocial. We have often heard from Primary
Health Care Nurses that there is never enough time for networking with the community providers who could potentially support their clients beyond the walls of their general practices.

This will be a perfect opportunity to meet these providers, collect resources, hear about how they may be able to support your clients out in the community and meet with other providers working
in Primary Health Care.

A DVD is available of the Nelson meeting held Nov 2008
This is an hour long DVD which has received excellent feedback from ME and FM sufferers.

This is a great resource for both patients and GPs – please feel free to copy!
