  • The Performance Management Programme:

The PHO Performance Management Programme (PMP) is a voluntary programme that has been designed by primary care representatives, DHBs and the Ministry of Health to improve the health of enrolled populations and reduce inequalities.

  • Capitation Enrollment and Registration Processes:

The Ministry of Health has recently updated information relating to PHO patient enrollment processes.

The Ministry's primary requirement of any practice is to achieve and provide accurate data for their enrolled as well as casual patients. As the calculation of capitation funding is derived from the data captured at source by the practice, it is imperative that processes and systems are in place to maximise the completeness and accuracy of the data associated with the individual.

You can find a really good overview of the enrollment process as well as enrollment forms on the following site: http://www.moh.govt.nz/moh.nsf/indexmh/phcs-payments-clientenrol
